Surgery (Radical Prostatectomy)
Potential Side Effects:
Urinary Incontinence: Many men experience temporary or sometimes permanent issues with urinary control. This can range from minor leakage to complete loss of bladder control.
Erectile Dysfunction (ED): Damage to nerves and blood vessels that control erections can lead to ED, with recovery rates varying widely based on patient factors and surgical techniques.
Bowel Issues: Although less common than with radiation, there can be some bowel-related issues if the surgery affects the rectum or nearby structures.
Lymphedema: Swelling in the legs or genital area if lymph nodes are removed or damaged during surgery.
Infertility: Removal of the prostate includes removal of seminal vesicles, which can lead to infertility due to the absence of semen.
Preparation:
Physical: Pre-surgery pelvic floor exercises (Kegel exercises) can help manage postoperative urinary incontinence. Consider sperm banking if fertility preservation is a concern.
Mental: Engage with support groups or counselling to prepare for potential changes in sexual function and body image. Educate yourself on the recovery process to set realistic expectations.
Radiation Therapy
Potential Side Effects:
Urinary Problems: Frequency, urgency, pain during urination, or even incontinence. Long-term, there might be risk of urethral stricture.
Bowel Problems: Diarrhoea, rectal bleeding, or proctitis (inflammation of the rectum). Chronic issues can include bowel strictures or persistent diarrhoea.
Erectile Dysfunction: While less immediate than with surgery, ED can develop over time, particularly with higher radiation doses.
Fatigue: A common side effect during and shortly after treatment.
Skin Irritation: In the treatment area, especially with external beam radiation.
Secondary Cancers: There's a small risk of developing cancer in the irradiated area years later.
Preparation:
Physical: Discuss with your radiation oncologist about potential dietary adjustments to manage bowel symptoms. Consider starting pelvic floor exercises.
Mental: Understand that side effects might peak towards the end of treatment and persist for weeks or months afterward. Mental preparation for possible lifestyle changes due to urinary or bowel habits is crucial.
Hormone Therapy (Androgen Deprivation Therapy - ADT)
Potential Side Effects:
Hot Flashes: Very common due to the reduction in testosterone.
Loss of Libido and Erectile Dysfunction: Due to lowered testosterone levels.
Bone Thinning (Osteoporosis): Increased risk of fractures due to bone density loss.
Muscle Loss and Weight Gain: Loss of muscle mass and potential fat gain.
Mood Changes: Including depression, irritability, or cognitive changes.
Fatigue: A common symptom as hormone level change.
Cardiovascular Risks: Increased risk of heart disease or diabetes.
Breast Enlargement (Gynecomastia): Can occur due to hormonal imbalances.
Preparation:
Physical: Engage in weight-bearing exercises to help maintain bone and muscle health. Discuss with your doctor about medications that might mitigate some effects like bone loss or hot flashes.
Mental: Prepare for changes in sexual function and body image. Psychological counselling or joining support groups can be beneficial. Understanding that these side effects are part of the treatment can help in coping.
General Preparation Tips:
Education: Learn as much as you can about each treatment's side effects. Websites from reputable sources like the American Cancer Society, Mayo Clinic, or Cancer Research UK are invaluable.
Support System: Build or lean on a support system that includes family, friends, and healthcare professionals. Support groups, either in-person or online, can provide emotional support and practical advice.
Medical Monitoring: Regular check-ups post-treatment are essential to manage or mitigate side effects. Discuss with your healthcare team about follow-up care and symptom management.
Lifestyle: Adopt a healthy lifestyle with a balanced diet, regular exercise, and adequate sleep to bolster your body's resilience against side effects.
Each man's experience with these treatments can be unique, so personalizing preparation based on your health profile, the specifics of your cancer, and your treatment plan is key. Always have open discussions with your healthcare team about what to expect and how to manage potential outcomes.
